うつ病は増えている？
日本では、100人に3～7人という割合でこれまでにうつ病を経験した人がいるという調査結果があります。さらに、厚生労働省が3年ごとに行っている患者調査では、うつ病を含む気分障害の患者さんが近年急速に増えていることが指摘されています。
「うつ病が増えている」の背景には、
うつ病についての認識が広がって受診する機会が増えている
社会・経済的など環境の影響で抑うつ状態になる人が増えている
うつ病の診断基準の解釈が広がっている
など、様々な理由が考えられます。
「うつ病」にはいろいろある
「憂うつな気分」や「気持ちが重い」といった抑うつ状態がほぼ一日中あってそれが長い期間続く、というのはうつ病の代表的な症状です。
こうした症状が見られた場合、うつ病と診断されることが多いのですが、本当は、これだけで診断がついたことにはなりません。
大うつ病と呼ばれるタイプのうつ病には一定の診断基準があり、参考になります。他に性格や環境、あるいはほかの病気やこれまで服用していた薬が関係していることもあります。
また、これまでに躁状態や軽躁状態を経験したことがある場合はうつ病でなく双極性障害（躁うつ病）であると考えられますのでそういう経験がなかったかの確認も必要です。
統合失調症などほかの精神疾患が背景にあって、抑うつ状態はその症状のひとつであった、という場合もあります。このような症状を万が一うつ病と診断されたら、本当の疾患が見逃されせっかくの早期発見・早期治療のチャンスをのがしてしまうことになってしまいます。
正しいうつ病の診断は、うつ病のどのタイプなのか、ほかの精神疾患である可能性はないか、などを確認することまで含まれるのです。
治療法にもいろいろある
うつ病の治療法は、一人ひとり違います。
典型的なうつ病ならば薬物療法の効果が期待できます。性格や環境の影響が強い場合は精神療法的アプローチや時には環境の整備が必要になります。ほかの病気や薬が原因の場合は病気の治療や薬を変えることを考えなくてはなりません。休職についても、休養が必要な場合とむしろ仕事を続けた方がいい場合もあってこの点でも方針はひとつではありません。
うつ病とひとくくりに考えて治療をうけるのではなく、うつ病にはいろいろあって、治療法もひとつではないことを知っておくことが大切です。自分のうつ病と、ほかの人のうつ病は違うものであり、治療法も一人ひとり違っていて当たり前なのです。
